Hybrid Therapy Model Transition Drives Significant Margin and Utilization Gains in Skilled Nursing Portfolio
Six-facility organization sees $1.4M margin improvement and doubled therapy utilization within 12 months

[Cumberland, MD] — Gravity Consulting today announced the results of a therapy model restructuring initiative across a six-facility skilled nursing portfolio in Louisiana, highlighting the impact of more integrated therapy operations on both financial performance and clinical engagement.

The family-owned organization transitioned from a traditional contract rehabilitation model to a hybrid therapy approach designed to better align therapy services with facility operations and clinical goals.

Within 12 months of implementation, the portfolio’s therapy program margins increased from approximately $1.1 million to $2.6 million—an improvement of roughly $1.4 million. During the same period, internal therapy-related revenues grew by 232%.

In parallel with these financial gains, long-term care therapy utilization across the portfolio nearly doubled, reaching approximately 30%–40% of residents each month. This increase reflects a more proactive and integrated clinical approach, where therapy teams worked more closely with facility leadership to identify residents who could benefit from skilled intervention earlier and more consistently.
